When you accept it will publish that information and the neighbour may then repay the favour which means bigger and better crops for you and more XP for having fertilised a neighbours crops and also having yours fertilised.

Gaining quick XP is valuable to levelling up therefore going into your neighbours farms on the base of your FarmVille screen and visiting their farms regularly (it will state that their crops are looking puny will you fertilise) will give you lots and lots of XP not to mention coins.
